MENTAL HEALTH THERAPIST INTERPROFESSIONAL PRIMARY CARE (IPC) MENTAL HEALTH TEAM
One (1) full-time permanent position - 35 hours/week

Annual salary: $ 63,063 - 75,979 $

The incumbent will always work according to the principles of health promotion and the determinants of health. They will offer psychosocial evaluations, assess needs in order to establish an intervention plan with the client based on clinical approaches, and offer individual and group interventions. In addition, they provide the appropriate referrals for clients with issues requiring long-term interventions.

RESPONSIBILITIES:

  • Provide assessment and individual mental health intervention services to clients of primary care providers in independent practice in the Niagara Region
  • Develop with the client an intervention plan according to his/her needs
  • Participate in case discussions and service planning with internal and external partners
  • Participate in the design and implementation of new programs to meet the clients ‘needs in the community: needs assessment, promotion and client recruitment
  • Organize activities aimed at raising awareness of mental health issues
  • Participate, as appropriate, in internal and community initiatives and committees aimed at improving the quality of life of clients with mental health issues
  • Participate in the development and application of intervention models that ensure quality services (recognized clinical approaches)
  • Liaise with community agencies to improve access to appropriate services in a timely manner
  • Prepare reports, maintain records and program statistics
  • Participate in team meetings, staff meetings and other relevant meetings
  • Participate in the clinical supervision program (individual and/or group)
  • Performs any other duties as assigned by the supervisor.

EDUCATION, EXPERIENCE AND SPECIFIC REQUIREMENTS:

  • Master's degree in social work, counselling or other university degree relevant to the practice of counselling and psychotherapy from a recognized institution
  • Member in good standing of a Professional College (OCSWSSW or CRPO )
  • A minimum of three years of relevant experience
  • Experience and practice in counselling and/or psychotherapy
  • In-depth understanding of mental health issues
  • In-depth knowledge of psychosocial assessment and counseling techniques
  • Ability to handle crises effectively
  • Knowledgeable on the mental health community resources available
  • Commitment to working co-operatively with clients and members of the interdisciplinary team
  • Openness to the customs and habits of other cultures
  • Excellent organization and interpersonal skills
  • Knowledge and ability to use basic computer software
  • Excellent knowledge of oral and written English (French language skills are an asset)
  • Be willing to work flexible hours to meet the needs of clients
  • Have access to a motor vehicle and hold a valid Ontario driver's license

La création d'un centre régional de santé communautaire en 1995 résulte de la fusion du CSC du Niagara, créé en 1992, et de celui de Hamilton. Sous une même structure et sous un même Conseil d'administration, le nouveau Centre, avec ses deux points de service (à Welland et à Hamilton) offre à la population francophone de la région des services de santé primaire, des services sociaux et communautaires ainsi que des programmes de prévention de la violence.

L'évolution du Centre de santé communautaire Hamilton/Niagara, regroupement à but non-lucratif financé par le RLSS de Hamilton Niagara Haldimand Brant, est donc le produit d'un effort collectif qui continue d’évoluer et de mobiliser de nombreuses personnes francophones tant dans la région de Hamilton que dans cell

 

The creation of a regional community health center in 1995 was the result of the merger of the Niagara CHC, created in 1992, and the Hamilton CHC. Under the same structure and Board of Directors, the new Centre, with its two sites (in Welland and Hamilton) offers primary care, social, and community services as well as violence prevention programs to the Francophone population of these regions.
